Tarsal Coalition/Ankle Sprain 2007

Question
Dr. Katz, Thank you for your time in reviewing my question. It is appreciated.  I am a 30 y.o. registered nurse, and I experienced an ankle sprain off of my Dansko clog 2x in one week. There was never any swelling after the events.  I tried to ignore the ankle pain and wrapped with an Ace bandage and continued to work at my full-time job.  I gradually became unable to bear weight over the next 3 weeks and went to the ER. There was no fracture found, hoever I was dx'd with a calcaneo-navicular coaltion which I never knew that I had.  An MRI and a CT scan were completed.  The abnormalities were listed as post tib tendonopothy and that was it. Nothing about the peroneal tendons at all.  I was put in a cam walker boot for 3 weeks, non weight bearing and told I had a "moderate ankle sprain.". I completed PT for 3 months and all the while complained of stiffness in my heel and abnormal amounts of pain with wt. bearing and prolonged standing. Told this was "normal" by the pt and the orthopedic surgeon.  I am desperate for help, I have pain on the outside of my foot and into the heel.  I have a strange sensation along the sural nerve area as well.  I have just started Accupuncture 3 weeks ago and have stopped doing the pt.  Do you have any thoughts?  I was told that I have limited ST joint motion, however this never bothered me before my ankle sprain. Do you know anyone in the Philadelphia area that does cryotherapy?  Thanks so much! -Joi

Answer
Hi Joi,

It sounds like you had a coalition and perhaps it was injured or fractured with your ankle injury.  It is common after an ankle injury to have sural nerve damage.  Sections of the sural nerve also go into the heel.

Here is a list of Penn. crysourgeons.  I don't personally know them.  However, I believe that cryosurgery would be agood option for you.  I would also strongly consider seeing a podiatrist for custom orthotic devices.
