QuestionQUESTION: I am a beginner racewalker who has been walking about walks about 15 miles a week UNTIL the top of my foot, near where it bends at the ankle, began hurting. May have been after I added toe raises (on the stairs) to my stretching routine but not sure. First noticed it on a usual 4 mile walk -- thought my laces were too tight. Was not the problem. Layed off walking for 3 weeks, tried ice & Advil. After 3 weeks still ached a bit but better. Tried an easy 2 mile walk, has been hurting a lot since. Can walk around house, work, etc. but not for long & no racewalking. I wish I could train & my new-found fitness level is slowly erroding. My podiatrist doesnt have an opening for 2 months. Any advice in the meantime?! Thank YOU!!

It is ridiculous to wait 2 months with pain. Find another podiatrist.
We are Board Certified in Surgery & Orthopedics and quite busy- and we do appointments within 1-2 days-if not same day. You are the patient and in pain!

Sounds like an irritation of the anterior tibial muscle of the foot. The toe raises probably pulled on the anterior tibial muscle insert and irritated it.
Google anterior tibial muscle so you can see how big the muscle is. When the muscle hurts it is called shin splints,but it sounds like you irritated its insertion on the top of the foot.
Have your gym shoes evaluated by a Podiatrist Board Certified in Orthopedics-not your running shoe store or OTHER non-medical professional. Might be as simple as the shoes

Protect-perhaps an ace wrap around the area of pain and limit the movements that aggravate it-Find a gym shoe that seems to decrease the pain on top of the foot
Rest-Speaks for itself. I do athletics and hate to take off more than 2 weeks.
Ice- As much as you can tolerate. Pros put the injured area into an ice bath for 15 minutes! Try ice packs instead.
Compress-Ace wrap or foot sleeve around the ankle
Elevate-Will help if it throbs at the end of the day.

Take max doses of anti-inflammatories for about 5-6 days-as long as you don't have any stomach problems.
If these suggestions don't help within 7-14 days then seek the aid of Podiatrists who are Board Certified in Orthopedics. Sometimes these injuries respond with orthotics and physical therapy or other modalities. X-rays may be necessary for the diagnosis.

QUESTION: What is the maximum dose of anti-inflammatory -- the dose on the bottle or more like 600-800 per day??
I wish I could come to your clinic -- 2 day wait is amazing!! Found out I can get into a local sports doc quickly but the wait for a podiatrist is still 2 mos. (unless they have a cancelation). I'm inclined to go to the sports doc now and follow up with the podiatrist later for further input (orthotics, shoes, etc.) Does this sound like a good plan to you? Thanks very much!!

The Prescription dose of Advil is 600-800mg three times a day-Prescribed by a Doctor.
Make sure you don't have any stomach problems please and consult a doc on those doses.
