QuestionDr. can you advise if conservative treatment would be appropriate for the following diagnosis:
Bone alignment and marrow signal is notable for grade I anterolisthesis of L4 on L5 with mild disc-related endplate changes also seen at L5/S1. At the L4/L5 level , no demonstrated spondylolysis in conjunction with the anterolisthesis, however, moderate facet arthropathy is observed. A bilateral paramedian disc herniation is noted. In conjuction with facet arthropathy and short pedicles, moderately severe centeral stenosis is observed with mass effect upon the ventral thecal sac and mild proximal bilateral L5 root mass effect.
At the L5/S1 level, diminished disc height is seen with an annular disc bulge wihtout lateralizing herniation, root displacement, or central stenosis with mild facet arthropathy.
I was recently told by a orthopedic MD that I need surgery to correct the problems. I've been seeing a chiroprator for over a year.
AnswerDear William, Thanks for reaching out. While there is a standard rule of thumb of not making a absolute recommendation based on radiological findings without considering the full examination findings it does seem possible that you may benefit from a conservative program of care much like I offer at Back Clinics of Canada.
Please take a moment out to see our site to better understand the approach by visiting www.backclinics.ca
As we have patients who come from all over the world to our centre you may consider visiting us to verify your candidacy and possible care.
Good luck,
Dr. Ron Nusbaum
