My first question is about pain pills. If my shoulder hurts, and I take a pain pill, how does the medicine "find" the pain area? Or does it stop the pain going to my brain? Which means also, how does the medicine go to the brain, or everywhere??
OK, you have a pain in your upper arm, but the pain ACTUALLY is in the ball-joint of your shoulder. Why does the pain appear to be in one place, but actually in another place?

When you take the pain pills, they get absorb into the blood system. From there they are transported all over the body. Where ever there is pain, the chemical in the pill will stop the pain's signal that has been sending to the brain. So in a way the chemical in the pill does not go only to the pain area but all over the body.
Since you have pain in the joint itself, there are other structures such as muscles, tendons,and ligaments surrounding the joint that can be affected by the injury to the joint.
