I can certainly empathize with your situation, but as a nurse I can tell you it's not a good idea to ask your healthcare workers to help you masturbate, it crosses the line of professional etiquette.  

I think you should discuss your issue with your sister, or another family member with whom you are comfortable talking and have them assist you to resolve your issue.  If you don't find a friend who is willing to help you, then you may need to look into using a prostitute for now until you get a girlfriend in the future.
