QuestionThis symptom has just started about a week ago. Everytime I get up from a sitting position, my entire left side of my face tingles and it last about 2 minutes. No pain is involved. My concern is why does the change in position activates this. I am a Physical Therapist and have explored many things but nothing is checking out.

Facial paresthesias can come from direct irritation to the facial or trigeminal nerve, or can be referred from upper cervical structures such as the tissue that connects suboccipital muscles to the meningeal membrane (there's a tissue bridge between the muscle and the dura). Other differential dx's include vascular pathology, since the transition from supine to sitting changes blood pressure. This could include a variant of temporal arteritis as well as cerebral aneurysm. If you get some manual therapy to the upper neck in the form of joint manipulation and soft tissue therapy, and your symptoms abate, then that would confirm a Ddx of mechanical irritation causing facial pain. You should also consult with your PCP for consideration of pharmacologic treatment as a way to see how your symptoms respond. Whether you see a DC or an MD for evaluation, it is imperative that you receive a good neurological exam. If nothing changes, or if your condition is getting worse with time, then you must consider a vascular etiology. MRI or MRA may be required. Not knowing anything about your past medical history (e.g. head traumas) or your biologic profile (e.g. are you 20 or 70 years of age) may not change how I'm thinking on this.
