About 6 weeks ago, I went to chiropractor who adjusted my SI joint by means of a side manipulation.  I  hurt my lower left back running eight weeks prior.  I went running and felt a sharp pain the next day in my lower back.  The sharp pain turned into a dull, achy pain on the top of my left buttock.  Sitting and standing for long periods made the ache hurt, as well as driving. I was diagnosed with a degenerative disc in my L5 Si disc 6 years ago and figured that I irritated the disc.  After 8 weeks of physical therapy, it did not get better, so I decided to try my luck at chiropractic.  The chiropractor did an exam with various tests and concluded that I my back wasn't the source of the problem.  He concluded that my SI joint was inflamed and raised a little, which is is why he did a side manipulation.  Long story short, I haven't felt the same since.  Immediately after the adjustment, I felt dizzy, but no pain.  As the day went on, my SI joint ached and my left calf began to tingle on and off.  It has been 6 weeks and I am still in agony.  Sitting, standing and walking for any length hurts, my SI joint is tender to the touch and my pelvis seems to be leaning towards the back because when I lay on my back, I feel as though I am laying on a bone that wasn't there before. Not only that, but ocassionally, I feel a tingle sensation in my left calf and left foot and my lower back seems to always be stiff. In the past 6 weeks, I have had an MRI.  The doctor found a 1-2mm bulge on the right side of my L5 S1 disc.   I have a couple of questions.  Can a chiropractor in one side manipulation move your pelvis so that it is tilting towards the back?  Can the chiropractor hurt your SI joint in one manipulation?  I am really uncomfortable and am worried that I will never heal up correctly.  I am afraid to see a chiropractor for fear that they will make it worse.  Can I do anything on my own to improve my situation?  Thanks in advance for your advice.

I am sorry to hear that you injured your lower back running.  I am sorry to hear that a trip to the Doctor of Chiropractic did not heal your problem, and may have aggravated your problem.

I will try to give you A LOT OF INFORMATION, and then you can make educated decisions about your situation.

"....About 6 weeks ago, I went to chiropractor who adjusted my SI joint by means of a side manipulation.....",- a very common complaint in the Doctor's office, a very common Chiropractic technique to begin to correct the subluxation.

".....I  hurt my lower left back running eight weeks prior.....", - eight weeks allows a problem a lot of time to deteriorate, degenerate, 'settle in', get momentum on it's side.  
              Unfortunately an aspect of injury is the process of degeneration or deterioration that starts with the trauma.  Over time, especially if left untreated, or treated incorrectly, a process of degeneration can take place.  Tissues change, chemistry changes, deterioration occurs-limiting the available potential return to health.
              I have a handout in my office that I give to my Staten Island Chiropractic patients about degeneration in the spine:


Degenerative    Disk     Disease

DDD = Degenerative Disc Disease
=  OsteoArthritis- THE most common Arthritis-  the 憌ear and tear disease?.                         

DJD = Degenerative Joint Disease; = similar/ same OsteoArthritis.

Degenerative Disc Disease  (old term =  DJD) is  actually  a  quite  common  condition.

  However,  the course of the condition   is  highly variable.   Some patients with DJD literally cannot walk, yet other patients with similar X-ray/ MRI findings may be able to run marathons or lift heavy weights.   Some people suffer for only a few days with pain and symptoms, some people suffer for months.

Although the course is highly variable, there are certain steps that are always helpful to follow:  Ice the area when painful-  10/ 15minutes on, 30/ 45 minutes off;  avoid certain postures and movements;  bend the knees when coughing or sneezing;  bend the knees when lifting anything;  do not extend legs straight out when sitting, laying down, lifting, or driving.

  Avoiding certain movements and postures will prevent aggravation of the condition.   Practicing good postures and movement patterns will help the condition heal and be less painful.  

DDD, DJD,  OsteoArthritis, -  deteriorating discs (disk)   is/ are  often caused by,  or often  concurrent with the Vertebral Subluxation Complex.  VSC is a misalignment which hastens deterioration of the joints involved.    Misalignment/  improper motion patterns  CAUSE/ HASTEN  OsteoArthritic degeneration.

VSC is a misalignment that hastens degeneration of the involved joints.  Joints are made to run for 100 years.  However, when a joint is misaligned,,, even a little bit, the wear and tear experienced by the joint is multiplied MANY times.  How many times have you heard of someone, after an injury to a joint, their medical doctor states unequivocally- "...the joint has been injured, you WILL have arthritis in that joint in the future (osteoarthritis, degenerative arthritis)...".  IF the trauma gives you arthritis, why do you not have arthritis immediately WITH the trauma?
              Is it something else?- is it a misalignment of the joint caused by the trauma, that OVER TIME causes the degeneration?  If we reduce the misalignment, will we reduce the degeneration, hence reduce the osteoarthritis?  Yes; reduce the misalignment, reduce the wear and tear, reduce the degeneration into osteoarthritis.

LEAVE the misalignment, cover up the pain with a pain medication or a 'pain patch', or a T.E.N.S. machine, cover up the pain caused by the misalignment, BUT LEAVE THE MISALIGNMENT allows the joint to deteriorate faster then it normally would/ should.

Spinal sprain/ strain/ injury often (usually) results in a misalignment.  This spinal segment misalignment is called a VSC:

         Vertebral   Subluxation   Complex    (a.k.a.  憇ubluxation?
The vertebral subluxation complex is the underlying cause of many healthcare problems.
A subluxation interferes with the proper functioning of joints, and can cause osteo-arthritic degeneration, Degenerative Disk Disease, degenerative joint disease, etc.
A subluxation interferes with the proper functioning of the nervous system (the master system which controls and coordinates all function within the body) and may cause various other conditions, symptoms and problems.



Subluxation  is  a  serious  condition  identified  by  its  five  parts:

    Spinal Kinesiopathology:
         This is fancy way of saying the bones of the spine have lost their normal motion and position. It restricts your ability to turn and bend. It sets in motion the other four components.

    Neuropathophysiology:
         Improper spinal function can choke, stretch, or irritate delicate nerve tissue. The resulting nerve  system  dysfunction  can  cause  symptoms  elsewhere  in  the  body.

    Myopathology:
          Muscles supporting the spine can weaken, atrophy, or become tight and go into spasm. The  resulting  scar  tissue  changes  muscle  tone,  requiring  repeated  spinal  adjustments.

    Histopathology:
         A rise in temperature from an increase in blood and lymph supplies result in swelling and inflammation.   Inflammed  Discs can easily bulge,  herniate,  tear,  or  degenerate.   Other  soft tissues  may  also  suffer  permanent  damage.

    Pathophysiology:
         The VSC contributes to  OsteoArthritic  degeneration.  Bone spurs and other abnormal bony growths attempt to fuse malfunctioning spinal joints. This spinal decay, scar tissue, and long-term  nerve  dysfunction  can  cause  other  systems  of  the  body  to  malfunction.


The Vertebral Subluxation Complex describes what happens when spinal bones lose their normal movement patterns and position.  When subluxated, joints are in a stressed, vulnerable, compromised condition.  Subluxation may cause Arthritis, Disk Herniation, Disk Degeneration,  or aggravate such conditions.   
          Vertebral  Subluxation  cannot be corrected through chemicals (medicine), stretching, yoga, vitamins or physical therapy alone.     Subluxation- a neuro/skeletal/muscular-   mechanical-   problem  requires  a   mechanical correction-  -  -    a manipulation,     best performed with the chiropractic adjustment.

An analogy to the degeneration (osteoarthritic development in our joints) is the front end of our car.  If we hit a bump and throw our front end out of alignment, our tires, and front end parts will degenerate faster then expected.  We can put the high priced gas in, we can put the high priced oil in, we can play the radio louder so we do not hear the wobble,,, but the parts wear out faster due to a mechanical misalignment.  Chemicals do not fix the mechanical misalignment; only a mechanical adjustment corrects the mechanical misalignment.


I think at this time, talking about treating symptoms (look at many medications, many treatments, the 'instructions' will tell you- "for symptomatic relief only"- meaning the CAUSE is NOT addressed!,  Here is another information handout I give my Staten Island Back Pain patients about 'covering up pain', chasing symptoms versus addressing the CAUSE of the problem:

Pain  is a  憆ed light on the dashboard?  

Pain is your body telling you that something is wrong.

That RED LIGHT on the dashboard of your car- -- do you ignore it until the car breaks down, or do you get it checked and correct the problem?  Pain, pain in your body- -- do you ignore it until your body breaks down, or do you get it checked and correct the problem?   Pain,  do you cover up the pain by taking a painkiller?  Take a painkiller, mask the pain, and allow a problem to progress in your body?


      NOTICE  ON  PAIN  RELIEVERS:
 
Label changes ORDERED by FDA;  the FDA announced proposed label changes for OTC over-the-counter pain relievers to  include the potential for stomach bleeding and liver damage (FDA news  206- 207;  12-9-06) ;

The  American Heart Association issued a scientific statement   recommending medical doctors change the way they prescribe OTC pain relievers from a first choice    to an alternate of recommending non-pharmacologic treatment (AHA statement  2-26-07).

NSAID Acceleration of ARTHRITIS; an important side effect of Aspirin and other NSAIDS is that it will inhibit cartilage repair and accelerate cartilage  destruction ( Journal of Rheumatology, 1982; 9: 3- 5 ).   Many times people take NSAIDS  for  the pain of Arthritis,  not realizing these drugs may make the underlying condition worse.   These medications  cover  up  the  pain,  and  cause  the  problem  to   worsen.



Pain  can  often  be  the  result  of  the  Vertebral  Subluxation  Complex.

Vertebral Subluxation  is  actually  a  quite  common  condition.   Doctors of Chiropractic look for pathological conditions which may require referral to other specialties,   and also look for 慡ubluxation? Other disciplines look for pathology, but overlook the importance of alignment and movement in the spine which affects our nervous system  (the master control system-                     ALL health disciplines learn this).  VSC- 慡ubluxation??can be the cause of many symptoms and conditions.



Only  a  Doctor of Chiropractic  will  evaluate  and  treat  for  VSC,   as  well  as  other  pathology.

You state;   "....The sharp pain turned into a dull, achy pain on the top of my left buttock......",- this is very common; the pain seems less sharp- because the nerves have adapted.
              Have you ever walked into a bar, restaurant, club- even a friend's party,,, and at first the music is sooooo loud- you cannot hear yourself think,,,,, but,,,,,, after a few minutes, your ears ADAPT, your nerves get used to all that volume, and you can carry on a conversation!
              Similarly, have you ever woke up late (well after sunrise), you take a walk to the front door, open the door, and you are BLINDED by the sunlight !!!!!, wait a minute, the nerves adapt- your eyes get used to that level of light.
              Often , injury is like that.  Pain diminishes, changes; NOT because things 'got better', NOT that things healed,,,--- the body just got used to it!

".....Sitting and standing for long periods made the ache hurt, as well as driving......."- certainly, certain activities, certain postures will aggravate a certain body part- and 'wake up' the nerves- a change has taken place!

"....I was diagnosed with a degenerative disc in my L5 Si disc 6 years ago and figured that I irritated the disc...."- so you have had low back problems before.  What did your doctor tell you then, what chiropractic maneuvers to re-align the spine were tried then?, hopefully you did not just take a chemical answer to a mechanical problem.

"....After 8 weeks of physical therapy, it did not get better, so I decided to try my luck at chiropractic....", the physical therapy was addressing the symptoms, not the cause.  Did you return to your previous Doctor of Chiropractic?, Did you go to a new chiropractic doctor (DC)?
What techniques helped in the past?

I am glad you 'decided to try your luck at chiropractic'.  Chiropractic doctors(DCs) pay much much less malpractice then Medical Doctors(MDs), most medical disciplines, and sometimes less then nurses!  Chiropractic is a safe, effective approach towards back pain.  Here are some statistics, websites that may interest you:

is  an  eye-opening  paper  published  as  a  Web  exclusive  in the March 2004 issue of Life Extension, a monthly magazine dedicated to "up-to-date coverage of the latest scientific and medical breakthroughs from around the world." As the grim title suggests, the paper explores the hazards of conventional medicine in the United States, analyzing and combining the complete published literature to date regarding injuries and deaths attributable to medicine.

Given those statistics, given that DCs pay much much less then medical treatment approaches for malpractice, you can see that chiropractic is safe, effective and cost effective.

"....He concluded that my SI joint was inflamed and raised a little, which is is why he did a side manipulation.....", the side manipulation is a common approach to the SI joint, as well as the L/sac junction and Lumbar spine.


"....Long story short, I haven't felt the same since.....", I hope you communicated this to your Doctor.  Just as MDs have very many different medications, different treatment approaches to a single condition; so do DCs have an array of treatment choices.  It is NOT uncommon for someone to feel sore after an adjustment, especially when the area is a problem area to begin with!
              What did your Doctor say when you spoke to him(her)?  Is this the Doctor you have been to before?  Have you been sore before?


Soreness is common, pain is not.  If the soreness or pain was markedly worse, certainly you should inform the Doctor.  As stated previously, there are many avenues to take on the road back to health, many treatment choices, just as at our medical colleagues offices.

A funny, but sad story taught us in Chiropractic school went something like this:
The Chiropractic Doctor is unlucky,,, when the DC makes a mistake, the patient is sore and tells everyone how chiropractic and that doctor failed him,,,,
The Medical Doctor is lucky,,, when they make a mistake, they get to bury their mistake, no one is left to complain.

I think we got that story from the architect school:
A Medical Doctor buries his mistakes, an Architect tells the unhappy client to 'plant vines'.


You state, "....Immediately after the adjustment, I felt dizzy, but no pain....", immediately after adjusting your lower back, you were dizzy?, this would be a rare reaction indeed.  Did you inform the Doctor?  What was the explanation?  This is a connection I find hard to understand.


"....As the day went on, my SI joint ached and my left calf began to tingle on and off.....", it is not unusual for the SI joint to be sore after an adjustment, especially after the first- or first few adjustments.  Adjustment is similar to exercise; while exercise may be the right thing to do,,, the body needs to ADAPT!, at first, exercise makes us sore, even if it is the correct thing to do!  Similarly, a soreness can come after an adjustment; but soreness is different from pain.
              You talk about left calf pain/ tingling; I am assuming the Doctor adjusted the Left SI ?

              You went to a doctor (DC) and are not happy with the outcome, why have you not contacted the doctor to complain, get an explanation based upon your specific complaints, you exam findings, your treatment and your response to treatment.  If you do not communicate with taht Doctor, then why not another Chiropractic Doctor to be evaluated and either referred appropriately or treated appropriately?

              In my Staten Island Chiropractic office I cannot tell you haw many MDs my patients have gone to, UNTIL they find one they are happy with.
              In my Staten Island Chiropractic office I cannot tell you how many DCs my patients have gone to, UNTIL they found me, are happy and stay for years for different complaints.
              And, I am sure, some patients have even left me- and found a different chiropractic doctor that they were more happy with !! (hard for me to believe)

In the past 6 weeks, I have had an MRI.  The doctor found a 1-2mm bulge on the right side of my L5 S1 disc.....", your low back is stiff; this is common with your condition; your joints are not functioning correctly- this may be indicating that manipulative adjustment should be part of your treatment regimen; LEFT CALF, LEFT FOOT, usually this does not correlate with a RIGHT HNP (herniated disc, bulging disc).

"...Can the chiropractor hurt your SI joint in one manipulation?..." yes.  Is this 'hurt' likely, no.  DCs have people in the side-lying position all day every day for work upon their SI joints and Lower Back.  Can a Medical Doctor hurt someone in one visit?- yes.  Can a dentist hurt someone with one cavity?- yes.


"....I am really uncomfortable and am worried that I will never heal up correctly....", I understand your concern.  With a diagnosis dating back at least 6 years ago of back problems, you have a long history of back pain, and need to try a regimen of care that you have not tried before.  If you keep doing the same things, you will keep getting the same results.  Have you tried years of medications?, Have you tried months of physical therapy?, and have you tried chiropractic, , , once?

"....I am afraid to see a chiropractor for fear that they will make it worse....", If you do not wish to see a Doctor of Chiropactic, then I would recommend seeing the Medical Doctor, perhaps a prescription could be of help in easing symptoms; a return to the physical therapist, perhaps some months of a different treatment then what you have had could help; perhaps a massage therapist or an acupuncturist.


"....Can I do anything on my own to improve my situation?..."- lose weight if you are heavy, eat an anti-inflammatory diet, stop smoking if you smoke, take appropriate nutrients.



When it bothers you a lot, perhaps this would help:


P.R.I.C.E. =    
P  protection.   R rest.   I  ice.   C  compression.   E  elevation.

Hurt  yourself ??  injury/ trauma/  bruise/ not even sure - - a broken bone ??

You should certainly get to the Doctor or trained health professional to be examined  if you think it  is  bad  enough.     BUT  in  the  meantime,  some  simple  steps  may  help.

  PRICE.

The  PRICE  protocol will usually help just about any musculoskeletal injury.

P = Protection;  protect the area from further injury- stop what you are doing, can you
     pad  or bandage the area ? Tape the area? Do something to protect the area !
R = Rest;  rest the area as much as possible, if a part is damaged, give the body time to
       repair the area
I = Ice;  ice is a potent anti-inflammatory; it will slow swelling, help with pain control
     (ice is analgesic), place ice over cloth over the injured area for 5, 10 minutes, off the
     injured area for 30 minutes; then back on again
C = Compression;  to help prevent swelling compression may be helpful- for example
      if it is an ankle, wrap the ankle with an ace bandage and ice the area.  Many body
      parts may not be applicable to compression.
E = Elevation; again to fight swelling and pain, raising the injured body part above the
      heart may be helpful- ie raise an ankle or knee, you lay down, raise the injured part

PRICE  may  help :    Sprains,  Strains,  musculo-skeletal injury,  bruise,  contusion
Sometimes  a slip, trip, fall, heavy lift, athletic injury, or a motor vehicle accident can cause a bump or bruise, sprain or strain.  Sometimes also involved with the injury is a vertebral subluxation (or other joint subluxation) (subluxation is a mechanical problem- of any joint- and requires a mechanical correction).  In the spine, a sprain/strain  is a misalignment,  a 慿ink?in the joint, a stretch of the muscles, tendons and/ or ligaments:- more properly termed a Vertebral Subluxation.   Sometimes symptoms can come immediately, sometimes days, weeks or even months later!- Well after the actual trauma !!!

         Spinal = Vertebral   Subluxation   Complex    (a.k.a.  憇ubluxation?  ;   ( subluxation COULD be any joint )
The vertebral subluxation complex is the underlying cause of many healthcare problems.
A  subluxation  interferes with the proper functioning of the nervous system  (the master system  which controls and coordinates all function within the body)  and may cause various other conditions, symptoms and problems.


This is a serious condition identified by its five parts:
    Spinal Kinesiopathology:
         This is fancy way of saying the bones of the spine have lost their normal motion and position. It restricts your ability to turn and bend. It sets in motion the other four components.
    Neuropathophysiology:
         Improper spinal function can choke, stretch, or irritate delicate nerve tissue. The resulting nerve system dysfunction can cause symptoms elsewhere in the body.
    Myopathology:
          Muscles supporting the spine can weaken, atrophy, or become tight and go into spasm. The resulting scar tissue changes muscle tone, requiring repeated spinal adjustments.
    Histopathology:
         A rise in temperature from an increase in blood and lymph supplies result in swelling and inflammation. Discs can bulge, herniate, tear, or degenerate. Other soft tissues may suffer permanent damage.
    Pathophysiology:
         Bone spurs and other abnormal bony growths attempt to fuse malfunctioning spinal joints. This spinal decay, scar tissue, and long-term nerve dysfunction can cause other systems of the body to malfunction.


The Vertebral Subluxation Complex describes what happens when spinal bones lose their normal movement patterns and position.
         Automobile accidents, improper lifting, improper posture, alcohol, emotional stress, chemical imbalances, and long periods of sitting can cause the Vertebral Subluxation Complex.
          Vertebral  Subluxation  cannot be corrected through chemicals (medicine), stretching, yoga, vitamins or physical therapy alone.  Subluxation- a neuro/skeletal/muscular- mechanical- problem requires a mechanical correction-  -  -    a manipulation,     best performed with the chiropractic adjustment.  For good health- Treat the   Cause,  not just the Symptoms.  Sprain/ strain/ bump/ bruise?= may result in subluxation of the nearby joint.  CORRECT the subluxation !!!!
