QuestionQUESTION: my wife complained of backpian when we were playing just after i pulled her towards me and she was travelling and the vehicle got in to pot hole which tossed her up and landed back on the hard surface eto the seat.
She could not move or stand so we took her to hospital and did a MRI. the below are the finding in MRI.
Impression :disc extrusion nated at l4-l5 level causing significant right neural foraminal
narroing and thecal compression.
Alignment is normal
Loss of disc hydration at L3-L4 and L4-L5 Leavels
Disc extruscion noted at L4-L5
The L5-si disc sapce is mildly recued with end plate degnerative chnages.
conus ends at L1-L2 level.
Now doctor treating her says the best way to cure is bedrest,pain killers and elctro physiotherapy.
they have also put her on traction now.
The pain is on the right lower back and runs down to her toes.
is this the best treatment, how soon she can get cure, how bad is the situation, what are other options to get release from pain and cure.
ANSWER: The best treatment is see a chiropractor who does Cox technique or has a spinal decompression table. Best rest for maybe a couple of days is fine, anti inflammatories for only a week or so. Ice packs 2 to 3 times per day for a week. I treat this problem everyday in practice and spinal decompression along with exercises (later on) is best.

Today we saw another Spine surgeon and he said he would not recommend anything other than a minimal intrusion surgery as he says the disc extrusion is too much and will not be a good idea of bed reset for more days.it is 9 days now from time of incident and she is recovered from pain about 50% in terms of frequency and 20 % in terms of severity.
I am looking for some good chiropractor in Bangalore. I will have to short list them and will consult soon. What are the chances of the cure if we visit the chiropractor and how long do you think this will help her.
what do you think about the surgery ?

AnswerIf surgery is totally necessary, then microdiscectomy is the way to go. If you can find a chiropractor of therapist that can do spinal decompression, you can have a good chance to recover. The disc is never cured once herniated, it is damaged. If the disc is ruptered and the material is extruded out against the nerve root, it may need to be surgically excised. The danger is when the condition begins to cause foot drop or weakness in the lower extremities, bowel or bladder weakness. Often, a steroid dose pack for taken for a week can really improve the condition. You can do conservative treatment for about 4 weeks, as long as there is no progressive significant weakness, before doing surgery.
