Question
The foramina are patent on both sides.

Straightened lumbar lordosis is seen.
L1-2 and L2-3: No disc herniation or canal stenosis seen based on sagittal images at these two levels.

L3-4: slightly asymmetric bulging disc is seen with small broad left lateral recess protrusion extending into the inferior aspect of the foramen, but without root impingement. Slight facet hypertrophy is seen.

L4-5: A moderate right paracentral disc extrusion is seen with AP diameter over 5 mm migrating slightly inferior in the right lateral recess to the upper L5 endoplate.  Focal sac and right L5 root compression is seen.  Mild facet hypertrophy noted.

L5-S1: Slightly bulging disc with facet hypertrophy is seen.

IMPRESSION:
1. Moderate right peracentral disc extrusion L4-5 with inferior migration. Sac and right L5 root impingement seen.
2. Small left foraminal protrusion at L3-4.

Answer
it is very tough to explain an mri in detail when i realy don't know your medical knowledge or vocabulary level. In plain english : you have a disc herniation at L4/5 disc causing pressure on the right nerve at L5 and a  smaller disc bulge on left side at l3/l4 level.
disc bulges can correct themselves over time and can also get worse over time. herniations are permanent injuries and will not heal or repair back to normal. many people live without pain and have disc herniations. If you need more advice on treatment methods please let me know.
